Registration

All new registrations for the Camas School District are taken centrally by the district registrar located in the Dennision Administration Center at 1919 NE Ione Street.

Below you will find a list of forms found within the district registration packet. For your convenience, you may print these forms, fill them out manually, and bring them with you to the Administration Center for registration.

It is necessary to schedule a registration appointment if your student is new to the district. Please contact Shanna Anderson, District Registrar, at 360-833-5410 to set up an appointment.

Additional Information Required

For preschool and elementary-aged students:

• State issued birth certificate
• Immunization information
• Proof of residency, i.e., driver's license, utility bill, purchase agreement, renter's agreement

For middle school:

• Copy of last report card
• Immunization information
• Proof of residency, i.e., driver's license, utility bill, purchase agreement, renter's agreement

For high school:

• Transcript
• Immunization information
• Proof of residency, i.e., driver's license, utility bill, purchase agreement, renter's agreement

If your student is on a 504 plan or an IEP (receiving Special Services support), please provide a copy that documentation at the time of registration.
